Turning Priorities Into Action

Building on the findings of the Community Health Assessment, the Community Health Improvement Plan outlines the priorities and actions that will help create a healthier Green County. Developed through community collaboration, the plan focuses on addressing key health challenges, including housing and mental health, and bringing local partners together to make a difference.

2025-2027 Community Health Improvement Plan

The Community Health Assessment informs decision-making, prioritizes health issues, and shapes an action-oriented, three-year Community Health Improvement Plan (CHIP). The CHIP is built on the voices of our community and focuses on the root causes of health issues by addressing community conditions and systems.

For 2025–2027, the plan focuses on two priority areas: Mental Health and Housing, with goals and strategies designed to support a healthier Green County.
