Understanding Our Community Health Needs
A healthier community starts with understanding the needs of its people. The Green County Healthy Community Coalition uses local health data, community feedback, and input from local partners to identify the strengths, challenges, and health priorities shaping life in Green County. These findings help guide decisions about where our community needs support and where we can make the greatest impact.
How the Assessment Works
Every three years, the Green County Healthy Community Coalition conducts a Community Health Assessment to better understand the health and well-being of local residents. The process combines community input with existing health data to identify the issues that matter most and establish priorities for improving health across Green County. 2024 Community Health Assessment
The 2024 Community Health Assessment brought together community perspectives and local health data to better understand the strengths, challenges, and health needs of Green County residents. GCHCC gathered input through a community survey, interviews with key community leaders, and conversations with residents, while also reviewing existing data from sources such as the U.S. Census and Wisconsin Department of Health Services.
By combining residents’ experiences with this broader data, the assessment identified key priorities for improving community health. These findings helped shape the 2025–2027 Community Health Improvement Plan, which focuses on mental health and access to safe, affordable housing.
“Giving support to activities and initiatives that help achieve our goals helps make Green County a healthier and happier place to live and work.”

The People and Process Behind the Assessment
FOSTERING COLLABORATION
The Green County Healthy Community Coalition (GCHCC) is co chaired by representatives from Green County Public Health Department and Monroe Clinic. GCHCC IS comprised of over 50 organizations representing local governmental agencies, community organizations, educational institutions and businesses.
IDENTIFYING NEEDS
An assessment process took place over six months, which incorporated components of primary data collection and secondary data analysis that focused on the health and social needs of Green County. The assessment also included the collection and analysis of input from persons who represented the bread interests of the community.
DETERMINING PRIORITIES
In addition to our assessment data findings, GCHCC also utilized Healthiest Wisconsin 2020 (the state public health agenda) and Healthy People 2030 (the national agenda) as blueprints to systematically identify Green County's health priorities.
The Green County health priorities, detailed in the 2025 2027 Community Health Improvement Plan, were determined by a facilitated group consensus method and are Mental Health & Housing.
From Assessment to Action
