The Green County Healthy Community Coalition (GCHCC) has completed the 2024 Green County Community Health Assessment. This is a process the coalition undertakes every three years to understand Green County's strengths and areas for improvement related to health. We asked for input from community members through a survey, community conversations, and key informant interviews. We'll also collected secondary data from sources like the US. Census and Wisconsin Department of Health Services. This information helped us determine which health issues our coalition needs to focus on so we can develop a plan to address them.

FOSTERING COLLABORATION

The Green County Healthy Community Coalition (GCHCC) is co chaired by representatives from Green County Public Health Department and Monroe Clinic. GCHCC IS comprised of over 50 organizations representing local governmental agencies, community organizations, educational institutions and businesses.

IDENTIFYING NEEDS

An assessment process took place over six months, which incorporated components of primary data collection and secondary data analysis that focused on the health and social needs of Green County. The assessment also included the collection and analysis of input from persons who represented the bread interests of the community.

DETERMINING PRIORITIES

In addition to our assessment data findings, GCHCC also utilized Healthiest Wisconsin 2020 (the state public health agenda) and Healthy People 2020 (the national agenda) as blueprints to systematically identify Green County's health priorities.

The Green County health priorities, detailed in the 2025 2027 Community Health Improvement Plan, were determined by a facilitated group consensus method and are Mental Health & Housing.
